A badge on a moving car does not wait at the starting point and then try to chase the car. It goes wherever the car goes.
The objective is not to make management chase every AI action after it happens. Josephine stays beside the consequential workflow so the work can be observed and protected while it is still moving.

Josephine may request that the release rate change from 100% to 25%. It may not cancel the claim, alter its value or invent another remedy.
If unresolved exceptions fall to 2, Josephine records the condition in the read-only Operator View. No SMS or external interruption is requested.
If unresolved exceptions remain above 3 after protection, Josephine may request the company-approved management notice. Duplicate notices for the same incident are suppressed.
The company decides whether protection continues and how it is released.
Josephine is installed beside a consequential workflow and follows the work through persistent work-item identity and company-connected evidence.
Josephine evaluates evidence from the work itself. It does not require the company to explain every AI agent before protection can begin.
Protection is limited to what the company has authorized. Josephine does not invent authority when evidence or permission is missing.
The purpose of protection is to keep consequential work from outrunning the company’s ability to understand the situation and decide what happens next.
The company determines in advance what protection is permitted for the workflow. Josephine can request only protection supported by that authority and by the available evidence.
Josephine keeps those states separate so management can see what the evidence actually supports.
If the evidence does not establish one of those steps, Josephine should preserve that uncertainty rather than report a completed intervention that cannot be supported.
Josephine distinguishes a developing condition from a condition that requires designated people to be brought into the loop.
A developing condition can be represented as an amber management status so the company can see deterioration without treating every signal as an emergency.
When a company-defined escalation condition is reached, the intended management path is to notify multiple designated stakeholders so consequential response does not depend on one person seeing one dashboard.
It needs reliable evidence about the work, a persistent way to identify that work, and a company-authorized protective boundary. When those are not sufficient, Josephine should say so.
That is the difference between trying to keep up with AI and creating a protective operating layer that moves with consequential work while management catches up.
